PROGRAM TERMS, RELEASE, AND REQUIRED CONSENT

Program Terms, Release, and Required Consent

Below are the legally binding Terms, Release, and Required Consent that you are agreeing to and providing when you complete an application to our programs on behalf of your son, as your son's parent or legal guardian, or as an adult to one of our adult-focused programs (i.e. Mission Trips).

Release and Consent For Minors Participating in any SJBC Program

As the parent/legal guardian of the above mentioned applicant, I hereby authorize the following:

  1. For the volunteers and/or staff of St. John Bosco Camps to consent to medical examinations and order medical treatment/medications for this applicant/these applicants at my expense. No further permission or notification is needed. (NB: we will make all reasonable means to contact the parents/guardians or the other indicated emergency contact in the event that we cannot reach the former.)
  2. For your applicant(s) to participate in on-site field trips sponsored and supervised by the volunteers and/or staff of St. John Bosco Camps.
  3. For this applicant(s) to participate in all sports and outdoor activities supervised by the volunteers and/or staff of St. John Bosco Camps.
  4. For the volunteers/staff of St. John Bosco Camps to administer any medications or prescriptions indicated in the health questionnaire according to the stipulated dose and directions, as well as any the non-prescription medication (e.g. ibuprofen, vitamins, cold remedies) at their discretion, including any medications prescribed or otherwise indicated by doctors in the event of an emergency, other than those specifically excluded in the heath questionnaire.

Consent to Participate, Medical Release, and Waiver of Liability

Medical Insurance Coverage: I acknowledge that neither St. John Bosco Camps nor the Camp Director, nor the Diocese in which the Camp is held, shall provide medical insurance coverage to my child. I assume all expenses for any medical treatment deemed necessary by the volunteers or staff of St. John Bosco Camps. Additionally, I acknowledge that all of the information provided above is current and correct.

Voluntary Consent to Participate: I, the undersigned, do hereby acknowledge that my child, the participant listed below, is physically fit and in a condition to fully participate in all camp activities and it is my wish for my child to participate in all camp activities.

Assumption of Risk: I realize that during the Camp, there are several ways that my child could suffer serious injury and be exposed to various sicknesses (i.e. viruses). During my child’s participation in the camp, my child could be exposed to: (a) strenuous physical activity (b) high temperatures, (c) physical contact, (e) and other dangerous conditions. The nature and extent of the Camp has been explained fully to me. I attest to the fact that the participant is sufficiently mature and physically able to participate in the Camp. I certify, to the best of my knowledge, that the participant has no physical/mental condition that will be aggravated by the program or that will impair the participant’s ability to participate in the program. In particular, we acknowledge the risk of serious injury and/or death from the program, including the following: (a) concussions and other head injuries (b) injuries from equipment (b) equipment malfunction (c) weather-related risks. I affirm that my child’s participation in the camp is strictly voluntary and I assume the risks associated therewith. I further realize that participating in camp may involve risks and dangers, both known and unknown, and have nonetheless elected to allow my child to participate in all camp activities.

Furthermore, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that we may be exposed to or infected by COVID-19 as part of participating in the Program. I acknowledge that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of ourselves and/or others, including, but not limited to, St. John Bosco Camps’ employees, contractors, volunteers, and participants. We agree to assume all the foregoing risks, waive liability against St. John Bosco Camps, the Priestly Fraternity of St. Peter, and the Diocese in which the camp is being held, and accept sole responsibility for any illness, injury, disability, or death, including all claims that may arise resulting from any of these, except for St. John Bosco Camps gross negligence or intentional misconduct.

Camp St. Isaac Jogues Applicants Only: We furthermore agree to release and hold harmless the Commonwealth of Pennsylvania from any and all liability arising from participation in the activity.

Waiver and Hold Harmless: I release and forever discharge and hold harmless the sponsors, organizers, volunteers, property site owners, and the Diocese in which the Camp is held, specifically including but not limited to, the Camp Director and all camp personnel and staff, as well as, St. John Bosco Camps, its affiliates, directors, officers, trustees, administration, employees, representatives, agents, successors and assigns (the “Releasees”), the diocesan staff and the Bishop of the Diocese in which the Camp is held, the Priestly Fraternity of St. Peter, and Our Lady of Guadalupe Seminary from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my from my child’s participation in camp except for in the case of St. John Bosco Camps’ gross negligence or intentional misconduct in connection with the program. This release specifically includes, but is not limited to any claim for negligence or negligent acts or omissions and any present or future claim, loss or liability for injury to person or property that my child may suffer, which arises out of my child’s participation in the Camp. Except as limited herein, I understand and acknowledge that this Consent, Release and Liability Waiver discharges from any liability or claim I or my child may have against Releasees with respect to bodily injury, personal injury, illness, death, or property damage that may result from participation in camp activities. I understand that the Releasees do not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health or disability insurance in the event of injury, illness, death or property damage. I understand and acknowledge that this waiver binds myself, my child, my heirs, administrators, executors, personal representatives, and assigns.

Medical Treatment: If my child is injured during their participation in any camp activities, St. John Bosco Camp’s and/or the Camp Director may render medical services to my child or request that others provide such services. By taking such action, St. John Bosco Camps and the Camp Director are not admitting any liability to provide or to continue to provide any such services and that such action is not a waiver by St. John Bosco Camps or the Camp Director of any rights under this release and waiver. Should my child require transport to a medical facility as a result of an injury, I am financially responsible for such transportation and medical treatment costs. If my child is injured during the program, it is my child’s responsibility to notify St. John Bosco Camps and the Camp Director immediately. I hereby hold harmless and agree to indemnify St. John Bosco Camps and the Camp Director from any claims, causes of action, damages and/or liabilities, arising out of or resulting from said medical treatment. I further agree to accept full responsibility for any and all expenses, including medical expenses that may derive from any injuries to my Child that may occur during his/her participation in the Program.

Marketing Photography Consent: I furthermore hereby grant permission for the use of my child’s name, photo, and/or video footage in connection with St. John Bosco Camps, including use on St. John Bosco Camps website. We have carefully read this agreement and fully understand its contents.

This RELEASE contains the entire agreement between the parties to this agreement and the terms of this RELEASE are contractual and not a mere recital. The information I have provided is disclosed accurately and truthfully. I have been given ample opportunity to read this document and I understand and agree to all of its terms and conditions. I understand that I am giving up substantial rights (including my right to sue), and acknowledge that I am signing this document freely and voluntarily, and intend by my signature to provide a complete and unconditional release of all liability to the greatest extent allowed by law.

As a parent or legal guardian of the applicant specified in the application (the Participant), I verify that my son has read this agreement and fully understands its contents, and I hereby accept these “Participant Consent and Release Terms” on behalf of my son.

As a parent or legal guardian of the applicant specified in the application (the Participant), I have carefully read this agreement and fully understand its contents. I give permission, take full responsibility for my son to participate in the Program, and hereby accept these “Participant Consent and Release Terms” on behalf of my son.

Code of Conduct for Youth Participating in St. John Bosco Camps Events

This Code of Conduct for Youth is a general list of rules and regulations for events conducted by St. John Bosco Camps. The lead chaperone or camp director is to ensure that each participant has reviewed and signed this form prior to any participation. Violations of any of these rules merit dismissal from the event at the discretion of the Camp Director and at the expense of the parents or legal guardian of the participant.

These rules and regulations must be followed at all events. The use by participants of drugs, tobacco, alcohol, pornography, fireworks, cigarette lighters or items that endanger people, pets, wildlife, or property is strictly prohibited.

  1. Profanity is strictly prohibited. Language and behavior should exemplify Christian values.
  2. Violent language or actions, bullying, and any physical attacks are strictly prohibited.
  3. Participants are expected to respect the rights and property of others. Vandalism and theft will not be tolerated. Financial obligations that result from such behavior will be the sole responsibility of the youth and his parents or legal guardians.
  4. Participants are not permitted to use any electronic devices, including but not limited to; cell phones, iPods, tablets, laptops, social media items, gaming devices.
  5. The only exception to this rule is the use of a camera which must not have internet or other network access.
  6. NB: participants are permitted to travel with cell phones. They must be turned into the Camp Director upon arrival at the event. Cellphones will be returned with sufficient time for charging or other arrangements prior to return travel.
  7. It is not permitted for a participant to be alone (i.e. out of sight and earshot, in a room or tent) with any adult chaperone, counselor, or volunteer.

These rules and regulations must be followed at all events, though it is foreseen and permitted that, for a particular reason, the lead chaperone or Camp Director may prudently decide to make exceptions to these rules and regulations.

  1. Clothing must be appropriate. Long pants and a suitable shirt (e.g. no tank tops) must be worn at Mass. Shorts are not permitted to be worn at Mass. References to drugs, tobacco, or alcohol products, including insignias or advertisements on clothing, or accessories, are prohibited.
  2. Participants are not permitted to leave the premises without the permission of the lead chaperone or Camp Director.
  3. Attendance at each event, and each part of each event, is required for every participant. Lack of cooperation in attending and participating may be justified by a reason deemed so by the lead chaperone or Camp Director (e.g. sickness, excessive tiredness, fear, discomfort, etc.).

Camp Participant Acknowledgement and Consent

As a camp Participant/Applicant, I have read this Code of Conduct for Youth and understand it, and will abide by the rules and regulations stated above. I will furthermore abide by all the rules established by the lead chaperone or Camp Director. I understand and agree that my parents or legal guardian will be notified if I commit any infraction of the Code of Conduct for Youth which requires my dismissal from an event, and that I will be sent home at the expense of my parents or legal guardian.

Parent/Legal Guardian Acknowledgement and Consent

I agree that my son, the Participant/Applicant, understands and is expected to abide by all of the rules and regulations as outlined in this Code of Conduct for Youth. I agree that if my son fails to abide by the Code of Conduct for Youth or engages in any infraction that is considered by the event coordinator or adult chaperone to be unacceptable, he will be dismissed from this activity and sent home at my expense with no right of reimbursement.

Acknowledgement and Consent for Adult Participants in Foreign Mission Trips

Mission Trip Consent to Participate, Medical Release, and Waiver of Liability

Medical Insurance/Travel Insurance Coverage: I acknowledge that neither St. John Bosco Camps nor the Program Director, nor the Diocese in which the Mission is held, shall provide me with medical or travel insurance coverage and am aware that St. John Bosco Camps strongly encourages me to purchase my own travel insurance. I assume all expenses for any medical treatment deemed necessary by the volunteers or staff of St. John Bosco Camps. Additionally, I acknowledge that all of the information provided in my application materials is current and correct.

Assumption of Risk: I realize that during the Mission, there are several ways that my I could suffer serious injury and be exposed to various sicknesses (i.e. viruses). During my participation in the Mission, I could be exposed to: (a) strenuous physical activity (b) high temperatures, (c) physical contact, (e) and other dangerous conditions on a constuction site. The nature and extent of the Mission has been explained fully to me. I attest to the fact that I am sufficiently mature and physically able to participate in the Mission. I certify, to the best of my knowledge, that I have no physical/mental condition that will be aggravated by the program or that will impair my ability to participate in the program. In particular, I acknowledge the risk of serious injury and/or death from the program, including the following: (a) concussions and other head injuries (b) injuries from equipment (c) equipment malfunction (d) weather-related risks. I affirm that my participation in the Mission is strictly voluntary and I assume the risks associated therewith. I further realize that participating in camp may involve risks and dangers, both known and unknown, and have nonetheless chosen to participate in all Mission activities.

By submitting my application I verify that I have read this agreement and fully understands its contents, and I hereby accept these “Mission Trip Participant Consent and Release Terms.”

Code of Conduct for Adults Participating in St. John Bosco Camps Events

This Code of Conduct for Adults is a general list of rules and regulations for events conducted by St. John Bosco Camps. The lead chaperone or Mission director is to ensure that each participant has reviewed and agreed to these guidelines prior to any participation. Violations of any of these rules merit dismissal from the event at the discretion of the Mission Director and at the expense of the participant.

These rules and regulations must be followed at all events:

  1. The use by participants of drugs, pornography, fireworks, weapons or any items that endanger people, pets, wildlife, or property is strictly prohibited.
  2. Profanity is strictly prohibited. Language and behavior should exemplify Christian values.
  3. Violent language or actions, bullying, and any physical attacks are strictly prohibited.
  4. Participants are expected to respect the rights and property of others. Vandalism and theft will not be tolerated. Financial obligations that result from such behavior will be the sole responsibility of the participant.
  5. Participants are not permitted to keep with themselves any cellular devices or other internet capable devices, including but not limited to; cell phones, iPods, tablets, laptops, social media items, gaming devices. The only exception to this rule is the use of a camera which must not have internet or other network access. NB: participants are permitted to travel with cell phones. They must be turned into the Mission Director upon arrival at the event. Cellphones will be returned with sufficient time for charging or other arrangements prior to return travel.
  6. Participants are not to engage in developing any romantic or "dating" relationship with other participants or volunteers of the Mission Trip, or with any persons met with or ministered to during the duration of the Mission Trip.

The following rules and regulations must be followed at all events, though it is foreseen and permitted that, for a particular reason, the lead chaperone or Mission Director may prudently decide to make exceptions to these rules and regulations:
